The LABF requires that its annuitants submit an original signature verification form each year. A second mailing of the 2024 Signature Verification Forms were mailed on June 11th. The forms must be signed by the annuitant, or his or her legal agent, in the presence of a notary and notarized. 

 

If you are an annuitant, please be reminded that the completed form must be returned to the LABF by July 15th to avoid interruption of annuity benefits. If you need a replacement form or if you have any questions, please call 312-236-2065.
