Each employee is required to complete a Membership Information Form and return it to the LABF. This form is a permanent record. The information that you provide will be used to determine your benefits; therefore, it is of the utmost importance that all questions be answered completely and accurately. The LABF also requires certain documentation to identify its members and to determine benefits. Please note that on the last page there is a list of all required documents.
